Form to Recommend a Lesson Plan to be digitized for inclusion in the KYVL Adult Education Resources Database

This form is used to submit a request for a lesson plan to be included in the KYVL Adult Education Database. The form is intended to give KYVL information about the relevance of the lesson plan and its suitability for digitization.  If the lesson plan is selected for inclusion in the database, a copy of the text of your plan will be made available on the KYVL.  We will contact you about getting an electronic copy of the full text, or a paper copy to be digitized.
